Type 2 Diabetes (T2DM) is a known chronic disease which could have severe complications. The project sought out a current evidence-based practice to help patients improve glycemic control. The purpose of this quantitative, quasi-experimental quality improvement project was to determine if the implementation of the American Diabetes Association's (ADA) diabetes self-management education and support (DSMES) program, utilizing short message service (SMS) would impact fasting blood glucose (FBG) levels compared to current practice among adult T2DM patients in a primary care clinic in North Texas over four weeks. Dorothea Orem's self-care deficit theory and Kurt Lewin's change model provided the project's scientific underpinnings. Data on FBG values (N = 12) were retrieved from the electronic medical record at baseline and four weeks post-implementation of the DSMES
